How to Apply

① Receipt from a designated testing facility which clearly shows your name and the amount of the PCR test

② Application & Request Form (Can be downloaded below, or picked up at city hall)

③ Photocopy of a form of personal identification (Driver’s License, Health Insurance Card, My Number Card, etc)

④ Photocopy of your bank book or cash card (Portion showing the name of the bank, branch, and account holder and the account number written in the Application & Request Form)　

☆ Submit ① through ④ to the Urasoe Healthcare Counseling Center (“浦添市保健相談センター” near the Urasoe Art Museum) by February 28, 2022 (Application by mail must arrive day of) ☆

*If the person applying is different from the person who received the test, a power of attorney form will be needed.

Power of Attorney form “委任状”

* In order to prevent the spread of COVID-19, we recommend submitting by mail. However, if the documentation is insufficient, necessary fields weren’t filled in, or were filled in incorrectly etc, the amount cannot be reimbursed. Please exercise caution when submitting by mail.
